  • For this week's Wednesday Checkup, I wanted to share a story with you about how a patient encounter did not go as planned in my office. I thought I did everything right but the outcome was not what I expected.

    In life, there are many times when we have good intentions where the outcomes are bad. During those times we need to evaluate why that happened so that we can avoid repeating the same mistake over again.

    I take it you are an OD. I am an MD and I do have respect for ODs, I have met many good ones. If someone comes to me demanding a special investigation or medication, my hackles are up immediately. I tell the patient he has come to me for my opinion, not for my subservience. Of course, some patients come to me from other doctors because they are, say, running out of chronic medication, such as metformin or levothyroxin. I prescribe it but I initiate a full work out, and an integral part is getting records from previous doctors and hospitals. I agree with your handling of this patient.

      I never filed a complaint against Dr. A (not representative of real name) either in written or verbal format. However I did transfer to Dr. B in the same practice due to a disagreement regarding treatment by Dr. A over a period of 2 years. Probably just my imagination, but I got very chilly looks for a while as I passed Dr. A's assistants to check in with Dr. B's assistants. Again, I only made a change and said nothing negative to anyone. I recognized that my perception of the need for difference in treatment was not necessarily a negative, thus no formal complaint was made. The advice given by another doctor to Dr. Mike is quite similar to the advice when trying to sever or change a personal relationship. It is odd that extra time and extra words can muddy some one-on-one situations.

                  You forget one thing: no one knows your intentions, even if you specify them, that might be interpreted as a lie. So given that intentions are never clear, if a patient doubts your intentions, you can never clear those doubts in a x minute interview.

                    Most of People if you do such recommendations they will always think you are trying to engulf money from them I don't know how much year of experience you have BUT all of the people (not what your experienced person has said) always value their TIME AND MONEY more then anything else no matter how you try to be good or else really having short conversations with them, some people will come by having their mindset filled with negativity about you some will come and put complaint against you. So value patient's time and money. If you were in India you would never have succeeded to become a doctor. it's harsh but it's true

                      I think you are wrong, your patient has depression and is in denial. He clearly does not have the energy to fight the system and just like you are pushing the blame on him he is pushing the blame on you and the society at large.

                      You should be able to identify this by the fact that he brings up a lot of stuff and actually cries and then also gives up. He might be very low, maybe even suicidal. Hes stuck in a job he clearly hates and probably feels there is no way out. I do not know how to solve this. Something needs to change with his job or find something else. You should summarize his symptoms psychological and physical together and at least give him some life advice. Maybe that is all it takes.

                                  Wow! I WISH I had those physical options after my accident. Sorry he treated you badly and didn't see the value in what you offered. You are treating the whole person, and mental and emotional health is part of that.

                                        You asked him about his life, he thought he could open up to you and vent (because we all have stressful lives and it feels good to vent) and you patronized him by trying to teach him something he and most other people already know. You gave him the kid's menu and crayons. Wake up.

                                              No you're thinking of Naturopaths. Osteophathic doctors are same as MD's they do the same training and testing. They just also learn osteopathic manipulations.

                                                  Hi Dr. Mike! I love you and your videos! However, on this one (omg, I can see the hate coming :P) I kinda understand the patient. Pain is pain. No matter whether there's a real physiological cause or if it's just psychological. I think sometimes doctors base their advice on certain premises or guidelines BUT when they actually suffer from let's say lower back pain (lol) they're OH FUCK! This realllllly hurts! Unfortunately, they didn't think that while giving advice such a reading articles. When you're in pain you'd like to cut it out asap (and yes, I know that's not possible in every case) but as human beings from the XXI century, we'd like a quick solution.
                                                  Having said that, I agree and understand where you're coming from and I APPRECIATE IT. Sometimes when you're in pain you just want a solution even if it's really hardcore :D

                                                    I remember when I tore my ACL, gave my symptoms, had the Lachmans test, Dr. did not request MRI and was sent to PT thinking it’s a knee sprain. After 6 weeks and still reporting instability dr still would not request it. Needed to change drs, but went to a sports specialist after four months of the injury was stating that I was fine even with the tear since I was back to walking at that point. Finally meniscus tore and I scheduled to see a surgeon who was like why didn’t you get this treated sooner. I do believe conservative is the best first treatment, but 1.5 years of wasted rehab time for a ticking time bomb in my knee

                                                        Everybody just wants to know what's wrong with them, relating it to stress often isn't a satisfactory diagnosis, however accurate.

                                                        Also maybe they just wanted a different doctor at the same practice but didn't want to talk to you about it, I mean my brother wanted to quit his job once but didn't want to tell his boss, so he just never went back 😂😂😂😂

                                                            with my single $3,000 deductible i'd be willing to try a lot of things before blowing it all on an MRI if it wasn't immediately indicated.

                                                                    I've only ever put in one complaint about a doctor. I had been told by my physician to go to the ER for what we thought might have been a blood clot (it wasn;t... those silly false positives). I waited all night for an ultrasound. Two doctors came in about 8:30 in the morning to tell me "they were not going to do the procedure on me and were going to send me home." I was so mad by their ignorance that I yelled at them both. "I'm here for an ultrasound, what the hell procedure are you talking about?" I've never seen two grown men run so fast. Never saw them again. But it sped up my wait to the test. Now imagine if I had been unconscious at the time. Lord knows what they would have done to me.

                                                                    Honestly, I would love to see a doctor like you who's so caring with a great bedside manner.

                                                                      That's what I hate about patients sometimes. They always want something to be done even if it's not indicated. But healthcare providers told me that healthcare is a business and you have to give patients something :'(

                                                                          I can understand how he thought books on mind over matter could be insulting. Like insinuating that the pain was all in his head. But everything else you said was great. And yeah a 9 mile hike is no joke.

                                                                            Did he get relief of symptoms after first surgery, if so for how long? Conservative options definitely the right move, however recommending that he follow up with his initial orthopod/neurosurg (if possible) would have been something to consider as well. If he's not looking to get another surgery MRI is pointless, because as you stated imaging does not always match the severity of symptoms.

                                                                                Dude stop these kind of videos. You litterly made an enemy for your community (I assume without you even wanting that). Yeah he is annonymous but still just look at the comments. I mean we have so little background story to this to not be able to judge but your fanboys will defend and attack anyone.
                                                                                Can anyone relate to the patient?
                                                                                If what he says is real maybe the patient is really tired to get new doctors over and over, maybe the stress you mentioned is just too long there because of the pain, maybe he thought this was the only ways he can do "something" becausr he feels helpless.
                                                                                I mean we have too little background so ppl will judge and hop on the fanboy side and we see that in the comments...

                                                                                  Hey doctor I'll tell you IMO what went wrong. The patient was fine till you mentioned mind, body reading material at that point he was like this guy is a foofoo essential oils kinda guy. Some people don't believe in mind body stuff and see that as non scientific , non medical spiritual foofoo. He went home and felt unsatisfied. And he wanted to see another doctor that would have offered him, medical remedies instead of natural remedies. Also people like that who request MRI's are just very thorough and have a perfectionist type of personality, he won't be able to sleep at night without getting that MRI, cause he needs to be sure. In his mind, it's just an MRI not a risky surgery or something he needs to worry about before going for.

                                                                                    Yeah people really underestimate stress. I was at a period of time in my job that was causes me high stress. That triggered me to get shingles. I still get stress from work but it's more manageable. Stay healthy, and protect yourself against stress.

                                                                                        Sounds like he wants meds. The other pressures in his life are what he is looking to quiet with meds. Just saying, the possibility is there. He should do stress management and maybe some meditation and yoga. Also important, retail is very stressful physically as you go from one thing to another to another very quickly and your physical well being is the last thing bosses consider. You are not the problem, doctor.
